New patients are always welcome. We will be happy to schedule your appointment upon receipt of your new patient paperwork. The form may be picked up in our office during regular business hours. Or, if you would prefer, you can download the new patient paperwork from our website.

We need to know all of the medications you are taking and their dosages.
Please bring all prescription bottles with you.
Also, if you have glasses or hearing aids, please bring them with you.

At your first appointment, we will need to make a copy of your Photo ID and Insurance Card(s).
We are not able to accept Medicaid. Medicaid patients need to be seen by their assigned providers.
Please arrive prepared to pay any deductibles, co-pays, or co-insurances that may be required by your insurance company.
We accept cash, check, and major credit cards as payment.
Patients arriving more than 10 minutes late will be asked to reschedule and charged a same day cancellation fee.
